摘要(Abstract)：

目的:探讨骨髓纤维化的临床特点、诊断、鉴别诊断、治疗进展以及预后。方法:实验室检查白细胞4.9×109/L,血红蛋白64g/L,血小板33×109/L,胸骨及其他部位骨穿干抽,骨髓滚片示骨髓见成堆异常细胞,拟诊骨髓转移性肿瘤;骨髓活检见髓腔内局灶中等大异型细胞,间质纤维化,网染+++,提示转移性腺癌。前列腺特异抗原明显升高。PET-CT示全身骨骼弥漫多发混合性骨质破坏,部分脱氧葡萄糖代谢轻度增高,标准摄取最大值3.6⁴.9。结果:予输血等支持治疗,入院第3天起患者全身不适伴有骨痛症状,予止痛治疗。因无法确定原发病灶未予针对性治疗,出院后1个月死亡。结论:诊断骨髓纤维化依靠临床特征和骨髓穿刺及活检检查,有待查清原发病因。
