目的：探讨末端可弯曲负压吸引鞘联合输尿管软镜治疗<3 cm肾结石的临床效果。方法 ：回顾性分析最大径<3 cm肾结石患者115例的临床资料，其中60例接受末端可弯曲负压吸引鞘（flexible vacuum-assisted ureteral access sheath,FV-UAS）辅助下输尿管软镜手术设为FV-UAS组，55例接受传统输尿管软镜（flexible ureteroscope,fURS）手术设为f-URS组。比较两组围术期指标、清石率及术后并发症发生率。结果 ：FV-UAS组手术时间[（60.57±4.98）min vs(63.00±6.96)min]、术后住院时间[（3.67±0.54）d vs(4.05±0.56)d]及双J管留置时间[（1.93±0.61）周vs(3.73±0.53)周]短于f-URS组，治疗总费用[（18 957.73±396.62）元vs(18 492.84±578.15)元]高于f-URS组（均P<0.05）。FV-UAS组即刻清石率（95.00%vs 87.27%）及最终清石率（100%vs 92.73%）高于f-URS组（均P<0.05）。FVUAS组术后石街形成发生率0，发热发生率1.67%，分别低于f-URS组的12.73%和10.91%（均P<0.05）。结论：末端可弯曲负压吸引鞘联合输尿管软镜治疗肾结石能有效提高清石率，缩短手术时间和住院时间，降低术后并发症风险。
